We accept most vision insurance plans. We will need your current vision AND medical insurance information when making your appointment. Insurance and discount cards must be presented when checking in. Full payment/co-payments are due at the time of service.

If you are a new patient (or if it has been more than three years since your last eye exam with us) you can save time by completing our Patient Registration form on the left.  

If you are unable to keep your appointment, please contact us at least 24 hours in advance.
