Why do we ask you for vision AND medical insurance information? In some cases, there may be a medical issue with your eye(s), such as conjuntivitis, glaucoma, or cataracts, where we will bill your medical insurance instead of your vision insurance. 

Full payment/co-payment is due at the time of service. All insurance and discount cards must presented when checking in for your appointment.
